By submitting this form: You verify that you have presented only LapSafe® products for this project and that you will not specify any competing product line or you will forfeit this project registration.
A partner that facilitates the sale of LapSafe® product(s) on a registered project is required to forward the end customer’s purchase order to LapSafe® in order for a pricing discount to be valid.
You agree that LapSafe® may contact the end customer specified in the application for information pertaining to the registered project.
